From October 1, 2026, federal Medicaid funding for many noncitizens ends, and that includes the Qualified Medicare Beneficiary program. CMS reminded practices today: a patient who leaves the QMB group loses the cost-sharing protection, but only from that date. Every earlier date of service stays protected.
CMS has rewritten the appeals chapter of its Medicare claims manual, effective December 11, 2026. The policy is unchanged, but the rules on late appeals are now plain: a physician practice gets good cause in two narrow situations, and a billing company that missed the date is never one of them.

On October 8, 2026, CMS told Medicare's claims system to go back over Part B claims from May 11, 2023 through December 31, 2024 that used place of service 32 while the patient was in a covered Part A skilled nursing stay. Starting December 1, 2026, Medicare's eligibility system can suspend an NPI's enrollment if the monthly email it sends to the practice bounces three months in a row. Every eligibility check sent for that NPI is then rejected until the practice fixes the address through its MAC. Your clearinghouse cannot fix it for you.
Your 2026 MIPS score sets your Medicare Part B payments in 2028, with a negative adjustment of up to 9%. If a cyberattack, disaster or other event kept your practice from collecting data this year, the exception applications close December 31, 2026.
